How to Obtain your Medical Records

 There are several ways for you to retrieve them.

 

1. Access them on our portal.  www.medicalofficeonline.com. If you need a new key or password, simply contact us us and we can reset it for you.(no charge for these records)

 

2.Send them to another physician: download and complete the release of records form. Be sure to identify the physician who will be receiving your records. The form can be mailed, faxed or emailed to us to us. We will then forward your records to the physician indicated.(no charge for these records

)

3. Have the records sent directly to you: download and complete the release of records form. Enter your name and address as the receiver of the records

 THERE IS A CHARGE OF $1.00 PER PAGE FOR THE FIRST 25 PAGES, AND $0.25 PER PAGE AFTER THAT. 

We will contact you with the amount due. Once received, we will forward your records to you. 

 

 

COMPLETED RELEASE OF RECORDS FORMS SHOULD BE FAXED TO 352-799-6722, OR EMAILED TO: STAFF@TOWNSENDGYN.COM
